Nope! Once you download and you enable the settings, no need to have the app open. RE Textblock will fitler messages without the app being open.
Apple does not allow filtering is iMessages. Text filtering apps (like this) can only filter SMS and MMS messages. The majority of spam comes from non-iMessage senders.
